Healthcare Professionals

“ChELO helps me connect with patients and understand what is important to them. I get to know about the experiences that make them who they are, and how this can guide our care for them now and in the future. ChELO makes me a better nurse, promoting patient focused care.”

6 questions could change everything

As a healthcare professional, you have ethical and legal obligations to patients at end of life. By asking just six questions, ChELO equips healthcare professionals to guide conversations about end-of-life care with patients and substitute decision makers.

ChELO ensures both healthcare teams and family members are informed and prepared for the patient’s end-of-life care.

Nurses, social workers, physicians and other frontline healthcare professionals using the ChELO are advocates for patient-centred decision making, and promote treatment and care patients want and can benefit from.

ChELO has been developed so that any member of the treatment team can initiate the ChELO process with patients and, for ICU patients, with the substitute decision maker.

You’ll get to know and understand your patient as a person, ultimately enabling you to propose treatment plans that honour and uphold your patient’s values, wishes and beliefs.
