As a healthcare professional, you have ethical and legal obligations to patients at end of life. By asking just six questions, ChELO equips healthcare professionals to guide conversations about end-of-life care with patients and substitute decision makers.
ChELO ensures both healthcare teams and family members are informed and prepared for the patient’s end-of-life care.
Nurses, social workers, physicians and other frontline healthcare professionals using the ChELO are advocates for patient-centred decision making, and promote treatment and care patients want and can benefit from.
ChELO has been developed so that any member of the treatment team can initiate the ChELO process with patients and, for ICU patients, with the substitute decision maker.
